Brain Perivascular Spaces Predict Cognitive Decline in Aging Humans

Researchers have demonstrated in living humans that dysfunction of deep venous perivascular spaces—fluid-filled channels surrounding blood vessels in the brain—reflects glymphatic system aging and predicts who will experience cognitive decline. The glymphatic system clears metabolic waste from the brain during sleep, and its failure is implicated in Alzheimer's and other dementias. This study used advanced MRI imaging to show that enlarged perivascular spaces in deep brain regions serve as a visible marker of glymphatic dysfunction and correlate with subsequent cognitive vulnerability. This is important because it provides a non-invasive way to assess brain clearance capacity in living people, potentially identifying at-risk individuals years before symptoms appear. Unlike animal studies or post-mortem analyses, this human evidence directly links an imaging biomarker to future cognitive outcomes. The findings support the growing understanding that maintaining brain waste clearance is critical for cognitive health in aging, and raise the possibility of monitoring glymphatic function as part of routine cognitive risk assessment or trials of interventions aimed at preserving brain clearance capacity.
